Case Management

When we say we provide More Than a Meal, we mean it.

Our care managment team is made up of social workers, a community health worker, nurses and occupational therapists to provide advocacy and support in many aspects of a senior's life.

Home modifications, managing medications and applying for Medicaid and Medicare are just a few of the things our team can provide. They also can connect seniors to other resources available in our community.

The YMOW staff and social workers work with students from the University of Michigan and Eastern Michigan University to see that every YMOW client receives a full assessment of their needs as service begins, and twice per year as service continues.

Once that assessment is complete, YMOW can pair seniors with a wide range of services and resources, including:

  • Access to our Personal Care Pantry
  • Help with pet supplies and pet care
  • Medical meals for renal and cardiac patients
  • Caregiver support for referrals to support groups and in-home services to care for their family member.
  • Help obtaining benefits such as Medicare/Medicaid and food stamps.
  • Medication review
  • Creation of a friendly visit program to combat social isolation.
