YMOW is proud to offer the CAPABLE program to qualifying clients of our meal delivery service.

CAPABLE - Community Aging in Place Advancing Better Living for Elders - is a program developed by nurses at Johns Hopkins University. Through regular visits with a team of professionals, seniors who participate in CAPABLE can gain more independence and live safer and more happily as they age in their own homes.

CAPABLE is available to YMOW clients who meet the following criteria:

  • 60 years or older
  • is a homeowner with homeowners insurance
  • has trouble with two or more activities of daily living (bathing, dressing, eating, toileting, etc.)
  • not currently undergoing cancer treatment
  • not currently undergoing dialysis
  • has experience no more than 2 falls in the past 6 months
  • has had no more than 3 hospitalizations in the past year

YMOW's CAPABLE team consists of:

  • an occupational therapist (YMOW Director of Occupational Therapy Clinical & Community Services Bethany Burge)
  • a nurse (CAPABLE RN Patty Tillman-Meakins)
  • a social worker (YMOW Social Worker Kim Armstrong)
  • a handyperson, also known as a Certified Aging in Place Specialist
  • the senior

Together this team will work with each senior to work toward changes that will help them remain safe, gain confidence, avoid falls, and ultimately postpone or avoid the need for residential care.

This free program takes place over about 5 months.
