CAPABLE

(Community Aging in Place—Advancing Better Living for Elders)

We are proud to offer CAPABLE to the homebound seniors of Ypsilanti and the surrounding area. CAPABLE is an evidence-based program designed by nurses at Johns Hopkins University to help seniors make positive changes to their homes and lifestyles, with a goal toward helping them to live more safely and comfortably at home.

After a break made necessary by the COVID-19 pandemic, nine area seniors completed the CAPABLE program in late 2020. A second group of YMOW clients started working with the CAPABLE team in November 2021. Through regular meetings with the team's nurse, occupational therapist and others, these seniors will emerge from the program with skills and home modifications to help them age safely in place.

YMOW is proud to offer the CAPABLE program to qualifying clients of our meal delivery service.

Developed by the Johns Hopkins School of Nursing, CAPABLE empowers seniors to live more safely and confidently as they age in their own homes. Through regular visits with a team of professionals, seniors who participate in CAPABLE can make changes to their lives and living environments that allow them to maintain their independence.

CAPABLE is available to YMOW clients who meet the following criteria:

  • 50 years and older
  • has trouble with one or more activities of daily living (bathing, dressing, eating, toileting, etc.) or 2 or more instrumental activities of daily living (housekeeping, shopping, medication management, transportation, meal preparation, etc.)
  • not currently undergoing cancer treatment
  • not currently undergoing dialysis

YMOW's CAPABLE team consists of:

  • an occupational therapist
  • a nurse 
  • a handyperson
  • the senior

With the support of YMOW's social work staff, this team will work with each senior to work toward changes that will help them remain safe, gain confidence, avoid falls, and ultimately postpone or avoid the need for residential care.

This FREE program takes place over about 5 months.
