Make a Referral

Meals on Wheels clients are primarily older adults or individuals with disabilities who are homebound and cannot prepare complete, nutritious meals for themselves. Clients are served regardless of age or financial status. The suggested donation is $3 a day, but no one is denied services due to inability to pay.

Eligibility

  • The need for meals should expand beyond a minimum of three weeks. However, shorter periods can be accommodated at the discretion of the Meals on Wheels Executive Director.
  • If a potential client anticipates the need for meals following a scheduled hospitalization, the referral should be made prior to the hospital stay.
  • Meals on Wheels clients must have a health status that generally prevents them from leaving their home for purposes other than medical appointments and occasional outings.
  • Meals on Wheels clients are unable to obtain food or prepare complete, nutritious meals because of their health.
  • Meals on Wheels clients do not have someone else in the home who is willing and able to provide meals.
  • A spouse or unpaid caregiver may also be eligible to receive meals if Meals on Wheels determines receipt of a meal is in the best interest of the client.
  • Health conditions prevent Meals on Wheels clients from participating in congregate meal programs, such as those provided by area senior centers and other groups.
  • Potential clients must agree to an initial assessment prior to receiving meals. Exceptions are made in emergency situations.
  • Clients must agree to a reassessment of their eligibility every 6 months.
  • Special dietary needs may not be met by the Meals on Wheels menu. It must be determined that use of the Meals on Wheels service will not jeopardize the health of a client.
  • Meal recipients must be able to feed themselves.
  • Meal recipients must agree to be home to receive meals and to follow YMOW guidelines for cancelling a delivery.

    Is the client unable to leave the home due to health or medical situation? Please, note lack of transportation or desire not to prepare meals doesn't make someone homebound (homebound needs are medical related). If you need a referral to a Senior Center we are happy to provide.



  • A doctor's note detailing the health and medical needs of the referred client is required for all referrals. Please send the supporting doctor's note to Ypsilanti Meals on Wheels via email at info@ymow.org or via fax at 734-482-3868 (this is a required document that is needed prior to the start of service).
    All clients except for Self pay clients must participate in a bi-annual health survey and assessment as is required by our funder the Area Agency on Aging 1B. This is a requirement for meal subsidy.

  • Required as YMOW is a partially federally funded program. Please list income by each client in the household (We will also need to know the entire household income).

  • Ypsilanti Meals on Wheels (YMOW) is only a partially funded federal government program. It costs YMOW $7.50 a day to deliver hot meal to ea. client, Any donation given is greatly appreciated so that we can maintain our mission to be a No Wait service provider for all homebound Seniors.

    Ypsilanti Meals on Wheels provides pet food assistance to clients in partnership with the Humane Society of Huron Valley's Bountiful Bowls program (once a month). If you have a pet in your household and you would like assistance please let us know.
